Our Service Navigators support adults with developmental disabilities before and after eligibility for Developmental Services Ontario (DSO) services has been determined. We aim to help our clients navigate the application process with supports such as registration, information provision and eligibility requests.
To apply for DSO services, clients must contact a DSO staff personnel and provide the requested information. Once eligibility is determined, some referrals and services become available such as respite, Passport funding, clinical services and Adult Protective Services Worker (APSW).
Once the full application has been completed, referrals can be made for certain Ministry of Children Community & Social Services-funded services, for which Service Navigators are the point of contact. These include residential, Community Participation Supports (post school programs), Family Support Worker and Person Directed Planning.

Helps to link you with resources and supports in the community - six agencies in Toronto, funded by the Ministry of Community and Social Services, work together to provide this service
Provides support for mental health and well-being of families and caregivers by providing a temporary break from the routine of providing care
