Specialized Programs & Services

In addition to our clinical treatments, Surrey Place also provides specialized programs and services that are customized and focus on addressing specific needs.

Each specialized program and service has specific requirements, unique admission criteria, and delivered using a combination of services, both clinical and non-clinical.

Community Network of Specialized Care (CNSC) – Toronto Region

The Toronto Network of Specialized Care (TNSC) is part of the Central Region Community Network of Specialized Care, is connected to Developmental Services Ontario Toronto Region and fulfills the Ministry of Community and Social Services mandated Service Resolution function for Toronto. Mashkikiiwininiwag Mazinaatesijigan Wichiiwewin (MMW) – Clinical Videoconferencing Program

Mashkikiiwininiwag Mazinaatesijigan Wichiiwewin (MMW) – Clinical Videoconferencing Program

The program provides specialized clinical services and resources for adults living with developmental disabilities. It also provides a unique opportunity for individuals, families, caregivers, and professionals to share resources and information over significant geographic distances.

Videoconferencing technology allows participants to meet via high speed, secure telecommunication links, in real time, face to face. Surrey Place, in collaboration with its partners, provides this service to communities situated in Northwestern Ontario.

Local agencies, professionals, and communities have access to specialized (therapeutic) services and educational opportunities, that may otherwise not be available due to cost and other factors associated with the distances from major centers.

Parenting Enhancement Program (PEP)

Parenting Enhancement Program (PEP)

The Parenting Enhancement Program provides parenting skills training and support to parents with developmental disabilities who have been referred to Surry Place from child welfare agencies, Adult Protective Services, Toronto Public Health and other community agencies.
Our clinical PEP Therapists work with parents to reduce the risk of child neglect and/or abuse and promote the social, emotional, cognitive and physical development of children. The program aims to improve parental abilities through a multifaceted approach which emphasizes individualized, home-based childcare instruction, education in child development, and advocating in support of the family.

Clinics

All the clinics require a referral from a health care provider. The consultation does not include prescribing and renewing prescriptions. Recommendations are made to the referring health care provider to continue the care of the individual, including medications and any diagnostic tests.

Adult Psychiatric Clinic

We provide consultation only service to adults aged 18 to 45 years old.

Family Medicine Clinic

Family Medicine Clinic

We provide consultation and physical health assessments to individuals 18 years and older.

Fetal Alcohol Spectrum Disorder Adult Diagnostic Clinic (FASD Clinic)

Fetal Alcohol Spectrum Disorder Adult Diagnostic Clinic (FASD Clinic)

The FASD Adult Diagnostic Clinic provides specialized diagnostic assessment and consultation to individuals 18 years of age and older with suspected Fetal Alcohol Spectrum Disorder (FASD) and developmental disability. This is the first FASD Clinic specifically for adults in Ontario.

Plus 45 Clinic

We provide care for seniors with intellectual and developmental disabilities (IDD) and support for their families and caregivers. The clinic specializes in supporting older adults with the challenges of aging, including the diagnosis and treatment of dementia due to its prevalence in this age group. We help with the transition to long-term care and help clients with advanced care planning and applying for palliative care. As a result, seniors with IDD report becoming better advocates for their health and families feel more supported.

Transitional Age Youth Clinic

The Transitional Aged Youth Clinic is a partnership between Surrey Place and the Departments of Pediatrics and Family and Community Medicine at St. Michael’s Hospital. Our goal is to improve the experience of the transition period for youth with IDD/ASD into adulthood. We provide guideline informed tools and resources developed by the Developmental Disabilities Primary Care Program (hyperlink to DDPCP website) (DDPCP) at Surrey Place to address transition planning. These tools address a broad range of needs in transition including legal issues around decision making in health, healthcare transition, vocational transition, social issues and family support, particularly around service navigation.

Young Adult Team

We provide clinical supports and services for young adults transitioning out of care, out of high school, and into the Developmental Services Ontario model.
