Each specialized program and service has specific requirements, unique admission criteria, and delivered using a combination of services, both clinical and non-clinical.
The Transitions Team supports people with complex transition needs through customizable, interdisciplinary paid services. The service is to support organizations with individual transitions for people with intellectual and developmental disabilities, including autism spectrum disorder (ASD), fetal alcohol spectrum disorder (FASD), and others. We offer services for all ages by preparing environments, staff, and caregivers with appropriate clinical services, tools, resources and education to ensure successful outcomes and prevent crises.
We work directly with professionals, agencies, and organizations. We start with an individualized consultation to explore all possible services. We carefully plan and facilitate client-centered services to transition individuals for successful outcomes. We will connect with you at every step to track progress and make plan adjustments to ensure goals are met.
The team may include ABA Therapists, ASD Consultants, Audiologist, Behaviour Therapists, Speech-Language Pathologist, Blind and Low Vision Care, FASD Consultant, Psychiatrists, Psychologists, Social Workers and many others.
Since our plans are highly individualized, cost per team varies depending on your needs. Payment plans are available.
