We use and share your Personal Health Information with external services and health care providers as permitted by law, as well as within your circle of care at Surrey Place to support the services you are receiving.
Depending on the situation, your personal health information may also be used to receive payments for services, teaching opportunities and to improve your services at Surrey Place.
What are your rights?
Access
Your Personal Health Information belongs to you. You have the right to request access to your information and a list of services or providers who received your information.
Correction
You can request to correct or complete your information if you think it is incorrect or incomplete.
Confidentiality
We have an obligation to protect your information from going missing, being stolen, and others sharing or misusing it. You can also withdraw your permission, and we will stop using and sharing your information.
