Our CNSC team serves adults living with complex intellectual or developmental disabilities who need to access multiple medical, behavioural, or justice-related services.
What to expect

Once you inquire, we’ll be in touch to start the registration process and connect you with your CNSC team.
Begin support
We work alongside you and your family, helping you manage situations that are too complex for standard services.
You and your family will:
- Receive help connecting with medical, behavioural, and justice-related services
- Get support coordinating referrals and communication between providers
- Have guidance until your ongoing supports can fully meet your loved one’s needs
- Feel more confident managing complex care situations
A confirmed intellectual or developmental disability is required. We can also support those who are suspected of having an intellectual or developmental disability, but a diagnosis must be confirmed within six months of service.
No, CNSC does not replace case management. It is an additional support for those with complex navigation needs.
