Coordinated Service Planning

As your service planner, we’re also your care advocate

The goal of coordinated service planning is to make care easier and more effective for children and youth with multiple or complex special needs and their families. We believe that each family has expertise on their child’s abilities and needs, that children should have a voice in their care, and that care must be inclusive and culturally-appropriate. These principles are ingrained into every interaction while we’re supporting you.

You’ll work with a Service Coordinator who will advocate for your needs and help coordinate services across Surrey Place and external systems, including:

• Health care providers
• Mental health services
• Behavioural supports
• Schools and education teams
• Child welfare agencies
• Developmental services
• Community agencies and resources
• Your coordinator will also monitor your family’s needs, adjust your care plan when needed, and bring in community service partners such as hospitals and schools when additional support is needed.

Who we support

Coordinated Service Planning is for children and youth who have multiple or complex support needs and need extra help coordinating services. You do not need a specific diagnosis to access this service. Coordinated Service Planning may support children and youth up to age 21 if they are still in school.

Note: If someone is 18 or older and new to Surrey Place, they may be connected to adult services instead.

What to expect

Inquire

Once you inquire, our intake team will learn more about your child’s needs and determine whether Coordinated Service Planning or another service is the right next step.

Assessment

Our team will explore your concerns, your child’s strengths and the services already involved in your care. This helps us understand what level of coordination your family may need.

Service guidance and support

An appointed team lead will coordinate communication and collaboration based on your family’s priorities. This ensures that client records are shared among all their teams, reducing the need to repeat the same information to different service providers.
