Help your loved one age safely at home

The Plus 45 Clinic supports caregivers of adults with intellectual and developmental disabilities (IDD) as their loved ones age. Our interdisciplinary team helps you confidently address changes in physical health, memory and thinking, mental health, and daily living.

If your loved one is experiencing age-related changes, the Plus 45 team can provide recommendations on medication, functional changes, and caregiver support.

  • Transitional Service Facilitator  
  • Clinical Nurse Specialist 
  • Behaviour Analyst  
  • Geriatric Psychiatrist

Who we support

The Plus 45 Clinic is for adults aged 45 and older with intellectual and developmental disabilities and significant age-related concerns.

What to expect

Begin registration

We’ll be in touch to start the registration process after you have been referred. If you have a referral letter, you can inquire online to get started.

Attend assessments

We offer hybrid assessments in-person or by telehealth, at community clinics, or at Surrey Place.

Work with our team

Your team will share practical recommendations and may collaborate with community partners to make sure you get all the support you need.

All clients must meet the following criteria:

Registered with or eligible to be registered with Developmental Services Ontario Toronto

45 years of age or older and identified as having issues related to ageing such as:     

  • Significant changes to the person’s physical health from baseline
  • Significant decline in memory and cognition  (suspected dementia)
  • Changes in neuro-cognitive or mental health status  
  • Decrease in activities of daily living
  • Multiple medications, or medications have not been reviewed in a long time
